Abstract:

Shigella is one of the etiologic agents of infectious colitis. The worldwide prevalence of shigellosis has been 14×107 cases per year. The important problem about it's causative agent is resistance to antibiotics.Martality rate due to shigellosis is almost high in young children, specially in developing countries. In addition, to WHO wright about anually determination of drug resistancy has been a strong motive in performing this study. The results showed that among 4042 cases suffering from acute diarrhea, 366 cases were culture positive for shigella, antimicrobial susceptibility test for 348 cases was performed and serotyping test for 366 cases showed that: 1 - The prevalent serotypes were I - Flexneri (56.55%) II - Dysenteria (42.36%).2 - 96.5% of cases were resistant to Ampicillin and 100% of cases, were sensitive to Cefixim. Otherwise the resistance to Co-Trimoxazole 63.7%, to Nalidixic acid 7.4% and to ciprofloxacin, 5.7%.It is concluded that the empirical use of Co-Trimoxazole and Ampicillin for severe cases not recommended, but Quinolones are the suitable drugs in Mashhad.

Abstract:

Background: In this study antimicrobial effectiveness test was performed on eye-drops which had high microbial contaminations in hospital practice to find out whether their antimicrobial efficacies affect the magnitude of microbial contamination during their uses.Materials and Methods: Artificial tear, atropine sulfate, betamethasone, homatropine hydrobromide, phenylephrine hydrochloride, phenylephrine zinc, pilocarpine hydrochloride, tetracaine hydrochloride and tropicamide eye-drops were subjected to the United States Pharmacopeia (USP) and British Pharmacopeia (BP) antimicrobial preservative effectiveness tests. Results: The results of this study showed that eight out of the nine products met the BP 'B' and USP criteria. The preservative employed in phenylephrine zinc eye-drop did not possess adequate antimicrobial activity against P. aeruginosa. Other eye-drops showed appropriate reductions in bacterial viability after 6 hrs, 24 hrs and 7 days, but showed a very low bacterial recovery after 28 days which didn’t comply with the no recovery (NR) term of BP 'A' criteria. Since viable microbial counts were usually determined by plate count method, it seems that the term of NR should define an acceptable range. Conclusion: The results indicated that there is not a clear correlation between antimicrobial efficacy testing of eye-drops and the rate of their microbial contamination while are being used. Other factors such as hygienic practices of eye-drops, proper bottle design and training of patients could influence their microbial contaminations. Regulation of in-use efficacy testing of eye-drops which is influenced by the environment, the frequency and technique of use, might be essential.

Abstract:

The basic premise of this research was to assess the ethnomedicinal uses of plants in an ancestral forest area at Naawan, Misamis Oriental and determine its inhibition effect against bacterial strains. The assessment of plants was conducted using the transect-plot method. Ethnomedicinal uses and the mode of preparations were obtained using a semi-structured interview questionnaire. Five ethnomedicinal plants: Crinum asiaticum L., Pavetta indica L., Bauhinia purpurea L., Mollugo pentaphylla L., and Cinnamomum mercadoi S. Vidal were selected for the anti-microbial test against cultured bacterial strains; the Escherichia coli and Staphylococcus aureus using the disc diffusion method. These species are commonly used by the indigenous people and known to cure stomach disorder and wounds. The mode of preparation of the extracts follows the traditional method of the indigenous people. Chloramphenicol, on the other hand, was used as positive control. Thirty-three out of 61 identified species were locally claimed to have medicinal value and are known to cure stomach disorders, poisoning, bleeding, cough, fever and wounds, among others. Antimicrobial test showed greater inhibitory effect of Pavetta indica and Bauhinia purpurea against Staphylococcus aureus and Escherichia coli, correspondingly; however, the synthetic chloramphenicol exhibited greater antibacterial action than any of the plant extracts. In areas where civilization is afar, importance of forest in terms of medicinal uses is highly acknowledged by the local community. Indigenous people, in particular, know the importance of the forest especially in relation to health care system and they believe that there is a need to conserve the resources to sustain the services it provides.

Abstract:

Contact dermatitis is an inflammatory skin disease due to contact with different substances and caused by two mechanisms: allergy and irritation. Contact dermatitis is a common skin disease and the most common occupational dermatosis. Differentiating between allergic and irritant contact dermatitis is essential in the identification of etiologic factor(s) and hence, its control and prognosis. This differentiation can be done only by patch test. Unfortunately, this simple, safe and useful test is always neglected. We review the basics of patch test, the method of performance, reading and interpretation of its results, possible side effects and results of studies done with patch test in different countries.

Abstract:

Producion of efficient antimicrobial drugs against to common human pathogens is one of the most important challenges in the field of medical microbiology. Continuous screening of clinical isolates and survey of antimicrobial resistance pattern is critical.Today, disk diffusion and dilution methods are known as the common antimicrobial susceptibility testing methods. Disk diffusion method does not render dependable results in determination of antimicrobial susceptibility pattern because this method is semi quantitative. MIC determination by microdilution and macrodilution techniques and recent method “E-Test”, have given interpretable results to researchers. Microdilution and macrodilution methods are also time consuming, expensive and error- prone methods. Whereas E-Test method is one of the means by which we will be able to determine susceptibility pattern of known species in low cost and short time. In this survey, constructing and printing of E-Test, coating of antibiotic on O.H.P film and quality control of constructed E-Tests were performed according to M7 A7 CLSI Standard. In this study quality of required materials for E-Test Production were suitable. Evaluation of constructed strips was performed using standard strain of staphylococcus aureus and reproducibly of results was appreciated.

Abstract:

The present study investigated varieties of C-test as measures of General Language proficiency (GLP). C-test was first introduced into language testing as a modification of close procedure. The original C-test was developed by mutilating the second half of every other word, beginning from word two ID sentence two of [l passage. Through various studies, C-test proved an integrative and a highly reliable and valid measure of (GLP. The present study was also aimed at further confirming those findings by investigating not only the original C-test but other varietis of C-test. To this end ten C-test versions were developed, by applying different deletion techniques and deletion rates, and were administered among nearly 500 senior EFL students, in the way that each form was given to almost 50 subjects. All the subjects had already taken a TOEFL. and a close test prior to the administration of the C-tests. The results of data analyses showed that C:-test may measure different things one of which might be general language proficiency and as such, not all varieties of C-test measure this construct to the same extent.

Abstract:

Background: The object of this study was to investigate the antimicrobial resistance pattern among common nosocomial Gram-negative bacilli isolated from patients with nosocomial infections. Methods: From June 2004 to December 2005, 380 isolates of common Gram-negative bacilli (Klebsiella, Pseudomonas, Acinetobacter and E. coli) from 270 patients with nosocomial infections in Sina and Imam Hospitals, Tehran, Iran, were evaluated for susceptibility to Imipenem, Cefepime, Ciprofloxacine, Ceftriaxone and Ceftazidime by Disc diffusion and E-test methods. Results: The most frequent pathogens isolated were Klebsiella spp. (40%), followed by Pseudomonas (28%), Acinetobacter spp. (20%) and E. coli (12%). The most active antibiotic was imipenem (84%). 26% of all isolates were sensitive to Cefepime, 26% to Ciprofloxacin, 20% to Ceftazidime and 10% to Ceftrixone. The susceptibility rates of Klebsiella to Imipenem, cefepime, ciprofloxacin, Ceftazidime and Ceftriaxone were 91, 25, 21, 13 and 7 percent, respectively and 91, 19, 17, 21 and 21 percent, respectively, for E. coli. Among Acineto- bacter spp., the susceptibility rate was 77% for Imipenem and 21% for Ciprofloxacin. Among Pseudomonas spp., 75% of isolates were susceptible to Imipenem and 39% to Ciprofloxacin. The comparison of the resistance status of microorganisms by both Disc diffusion and E-test methods showed a clinically noticeable agreement between these two tests. Conclusions: Since antibiotic resistance among Gram-negative bacilli has increased, enforcement of policy regarding proper antibiotic use is urgently needed in order to delay the development of resistance. Although it is widely accepted that E-test is more accurate in determining the resistance of microorganisms, our study showed that the Disc diffusion test will give the same results in most occasions and is therefore still considered useful in clinical practice.
